By submitting this link you hereby acknowledge the information provided on the registration form to be truthful and to best of your knowledge and that you will be held responsible for any false information given. As well that you have read and agree that you understand the policies and contract explained here.
(This is a contract for which you will be held responsible to pay.)
Student must register to attend either an AM (morning) or PM (evening) class. Student will also be assigned to attend the facility-based clinical training hours corresponding to the class that is chosen, with exception of either by choice (as available only and prearranged) or if asked by a scheduler to attend the other hours based on attendance numbers, placement and the availability of clinical spaces.
Class ending time can fluctuate and is dependent upon lecture content and skills demonstrations duration.
Student must be available for the schedule provided by the school for both theory and clinical training hours. Student does not have a choice of days of week to attend. The facility-based clinical dates are provided at the beginning of training session and student must make arrangements to meet the obligation of attendance to (40) clinical hours. There is no make-up time during the session and if student is not available or is absent they will be rescheduled to attend during another session as space becomes available. One offer to reschedule will be made with the student. Failure to comply ends the commitment by the school and training will be considered incomplete and fees incurred by the student to return at another time. Fees apply for class & clinical make-up hours/days in another session.
When registering, either on-line or in-person at the school, the registration includes both the theory and clinical portions of training. Classroom and skills practice labs will be held at the school for a minimum of (12) days and there will be (5) 8-hour facility-based clinical days at an affiliated healthcare facility of the school. Class hours are either 10:00 to 2:00 p.m. or 4:00 to 8:00 p.m. with corresponding clinical of either 6:00 a.m. to 2:00 or 2:00 to 10:00 p.m.
'Ability-to-Benefit' Registration Assessment:
When registering on-line the student is required to contact the school at 425-257-9888 to arrange a visit to complete registration paperwork as well as an ability-to-benefit assessment of the student’s ability to read, write and comprehend the English language. Student will be responsible to complete upon arrival on the first day of class and must pass this assessment in order to continue if they have not done so prior to the start of the session.
Terms of Use of PayPal:
Payment is accepted through PayPal as a courtesy to students for reserving a seat to attend class.
The MEDPREP contract supersedes and voids any policy that PayPal may have regarding refunding including the ability to initiate a dispute. Such an initiation will not be recognized.
Upon registration, either in-person at the school or on-line, student is liable to comply with the schools regulations involving payments, attendance, withdrawal and refund policies. PayPal is only an on-line transaction processor and is not a part of the school's policies or contracts. Payment through PayPal using credit / debit cards creates a binding contract upon registration and allows for a five day grace / cancellation period. You are affirming that you are in agreement to and understand these terms.
Withdrawal / Refunds:
In compliance with WAC 490-105-130 refunds are 100% if the applicant cancels within (5) days of signing a contract or making the initial payment as long as the applicant has not started training. Once the applicant has started the program the school can retain the reservation fee (deposit) and appropriate percentage of tuition. And in the event a refund is due (after the start of class) it will be processed within 30 days from the official withdrawal notice date.
Student must provide a signed & dated written notice of intent to withdraw within the five-day withdrawal period of entering into the contract whether registered in-person or payment made on-line. The written notice can be on-line by email to [email protected] if not available to provide at the school and the student’s full printed name will take place of the hand-written signature.
A phone call does not initiate or complete a withdrawal process, and the withdrawal will not be initiated if not received in writing, either by in-person at the school or by fax, USPS mail (within the five day withdrawal period) or through an email including the full printed name, date & time of the withdrawal.
5 Day Withdrawal Process:
In the event of a cancellation / withdrawal the refund will be at 100%, less the fees charged for transactions when done on-line, if initiated within five days of registering. The school is not responsible for transaction fees. The reservation fee is nonrefundable after the fifth day post registration for a session and there is no credit given to another session.
Miscellaneous Billing:
Additional billing can be initiated and presented to the student if school staff has to invest time above and beyond the normal business regarding paper processing, concerns regarding criminal backgrounds, duplicating, postage, or providing documentation to any agency, company or governing entity, or the time involved in a dispute. The student can become responsible for any and all expenses incurred by the school / staff and be assessed an hourly rate for compensation once enrolled or even if no longer a student.
In the Event of Theory Failure or Incomplete Training Hours:
In the event a student does not pass the theory portion of training with the minimum percentage score required other steps will be offered. If the student is non-compliant and/or doesn't attend as pre-scheduled the training will end immediately. Returning can then initiate an hourly or assessed rate to return. Other assistance can be: additional written tests, more skills practice attendance and postponement of the clinical training hours. When additional time is requested by the school program director during training there is no additional charges to attend the prearranged training hour period.
Religious Accommodations Policy:
The MEDPREP Vocational Training Center will make good faith efforts to provide reasonable religious accommodations to students who have sincerely held religious practices or beliefs that conflict with a scheduled course/program requirement. Students requesting a religious accommodation should make the request, in writing and preferably by email to [email protected] to the attention of the program director with as much advance notice as possible. Being absent from class or other educational responsibilities does not excuse students from keeping up with any information shared or expectations set during the missed class. Students are responsible for obtaining materials and information provided during any class missed. The student shall work with the program director, or the school administrator on her behalf, to determine a schedule for making up missed work.
Examples of religious accommodations will include: rescheduling of an exam and/or the assignment due date different from that given to the other students. There will not be any consideration for extra-credit to take the place of or the substitution of any closed-book test or the actual assignment that is required. Due dates or testing accommodations must be pre-arranged and agreed upon by both the student and the program director (or school administrator) prior to the testing or dates in question. Due dates or the accommodation for testing will only be available / scheduled on what would be the normally scheduled open class dates or by appointment availability of the school administrator.
Scheduling time may not be available during the original session of registration and require that the student return to another training period which would postpone graduation. Should the student default and not attend the (pre-scheduled) make-up date / time the school will not be liable to reschedule and the grade for the missed test / assignment will be according to the school’s grading policy. It will be the responsibility of the student to inquire further. Any additional consideration will be given on an individual basis.
Disclosure of Criminal Background:
Student is required to have processed both a Washington State Patrol inquiry and DSHS inquiry according to the facility clinical assignment. Full disclosure of any prior, current, or pending crimes/ violations including dates of occurrence /closure is mandatory. A rejection prevents attendance to clinical training hours. False information results in termination from the program.
